What is the purpose of the survey? What do you hope to learn from the answers? What will you change about your next step based on the answers that you receive?
Why should the people answer your survey - what's in it for them and how do they know that?
I've actually managed to read all three versions of the survey before actually getting around to replying.
I like the last one best. The only thing is in section 4 I wouldn't answer the questions about how I feel about Soundsmith Fitness before actually trying your classes. Since it's a required question that would probably kick the whole survey out. I would either eliminate those questions, make them optional or add a undecided option for answering.
How about this: Section 4 is really where I'm hoping to glean some relevant data from everyone who takes the survey, hence the required questions, so what if I ditched the branding and just put the generic "youth-oriented fitness classes" or something back in its place? That would still work within the purpose of the survey since...

While I'm developing the survey online, the initial rollout will be paper-based with SASEs in mailboxes around my neighborhood since (a) there are a LOT of families, (b) this really confines my geographic reach to start so that (c) logistics would be simpler since everyone could just go to the park at the end of the neighborhood for classes, plus (d) I'm leaning on word-of-mouth in the area once the first people sign up, which will (theoretically) make growth a little slower and manageable.
Completion of the survey entitles the respondent to a free class that is shorter than a normal class (a half-hour versus an hour) with a short presentation/Q&A at the end. Again, it'd be at the nearby park, so it wouldn't be an undertaking for neighborhood people to go there. As the survey says in a couple places (if not already, it will), mailing in the survey with contact information gets them that voucher, which may or may not have a list of class times to choose from (I'm thinking more about not doing that and instead setting up something directly, though that runs the risk of (a) low infomercial attendance and (b) the need for a lot more freebies). I'm in initial talks with someone about getting a website up and running semi-soon, which I could then point the respondents to and have them just select an open time from the calendar on the site (interactive so that I have a dynamic spreadsheet showing me how many people have signed up for which sessions, letting me see which times get more responses so that I can drop some sessions as needed).

Those would be great for a customer satisfaction survey but I'm not sure how you could ask these about youth fitness oriented classes in general. I suppose you could ask something along the lines of how much would these values affect your decision to purchase youth oriented fitness classes or have them rank the importance 1-5? The only problem with that though is I don't think you're going to get useful responses there as I can't imagine someone not wanting those things. Maybe you'd get useful responses if you asked them to select the most important value????

Trackin' now, and a good point. I imagine that in a preliminary survey such as this, everything would skew toward the higher end of those spectrums (except for cost, of course). Consider this (a still shorter) version number four.

I think you track pretty good. I like the last survey. It's short and to the point and it seems like you'd get the information you're looking for. You have an awesome way with words. It would have taken me a long time to come up with a way to phrase that question.

--Pleased with the KB jerks. Seems my problem shoulder is learning its place more and more.
--The walking lunges were done one leg at a time, as in right leg and right arm waiter's carry for five reps, then left leg and left arm waiter's carry for five reps.
--A rope climb "rep" is really counting foot pinches, each of which may contain 2-4 arm pulls, so in this case, I ascended as far as three non-slip foot pinches got me for each set.
--So much user testing feedback on the survey. I can't thank y'all enough. Lots of stuff I would've never thought of. Of course, as I'm rolling toward deployment day, I was not-so-subtly told of a job opening coming up that would have absolutely zero to do with training. Instead, it'd basically be the A/V tech job I had back in college, just salaried with (state government) benefits, free college tuition, and some management responsibilities. Plus I'd most likely be reporting to my former boss there, who I LOVE and still hang out with a lot. The only drawbacks would be take-home pay and hours (often irregular). Of course, I could still do the training thing on the side for extra cash, just make sure the training calendar is setup at least a month out so people could plan around my schedule. Gah.

--I'd planned on ten circuits, not five, but I'd also forgotten how much that RDL/lunge superset yesterday tends to surprise you a day or two afterward. SOREOMGSORE!!!11!1 I stopped hurting by the end of the first circuit, but there was almost no strength worth noting (not that there ever is), so I was sucking wind hard by the fourth set. There's also supposed to be a core circuit at the end of this one, but that ain't happening either. I'll definitely get that in tomorrow since I don't have a major workout planned.
--Going to see Phantom of the Opera tonight. First time in years that I've gotten to see live theater, so I'm looking forward to it. Sucks that tickets are usually so expensive (got this at half-price through a friend) since there's always something playing that I want to see. Oh well, time to freshen up the head shave.

We LOVE Phantom - have fun!!! We've seen it twice (many years ago) - once near home and once in Toronto... and we're so happy that it's coming back around again so we can take our kids this winter. Yay!
I was totally going to workout today, then I totally got fired, so I totally have no motivation to workout. That's not to say it's a bad thing that I got fired (over something completely unrelated to work, mind you), but my head's still a little swimmy. Will (hopefully) jump back in tomorrow.
Someone mentioned taking my youth fitness stuff and approaching *shudder* my old gym to see if they'd want to work it into their class schedule. Worth a shot, but I'm going to wait until next week to start looking at work options. Until then, it's Xbox360, workouts, and cleaning.
And Phantom was awesome, just a bit cumbersome on a belly full of pizza and beer

I probably should mention that it wasn't at all like this every day or even every month. He'd have his days or weeks of bitchiness, but 95% of the time, he wasn't around in any way, shape, or form, so I wasn't really in a constantly bad situation, just a constantly tense one because no one ever knew what kind of mood he'd show up in, if he showed up at all. I think the unknown aspect is worse than someone constantly being a prick.

So, Ben, if you're thinking the angle of going to your old gym yet seem loath to, why not other gyms in the area? Why not multiple gyms? The chick that taught yoga at my old gym taught 2 sessions a week there, a couple sessions a week at another gym, and teaches at the local yoga studio. (Plus does home sessions, 1 on 1 work, etc… but you get my point)
It then seems like something you can stop if you want, or going on your own doesn't take away from that line of work (I got my yoga sessions at the gym as "free" with my package, but I'd have to pay if I wanted to take her class at the studio, and the gym loses nothing really, because people who weren't members couldn't take the classes offered there)
I was reading an article about an upscale hotel gym in the area (Renaissance ClubSport ) and the programs that they offer for kids - seems play oriented. The article had more specifics but it isn't available online. I'm thinking that they might be open to an inquiry of how their programs work?

Thing with my gym is that it's literally the only game in town. There are maybe two other franchise gyms that have a few locations, but my old one covers at least a fifty-mile radius, though classes may be confined to individual gyms.
As I mentioned in the long-windedness of my blog (and expect no one to really read), I don't want to jump into anything major just yet until I get a feel for how that one job is going to pan out. I do have a couple months to play with, so I'm not panicking just yet (I'll save that for a month down the road when I'm moving and realize I still have no job ). Plus, the more I can put it off--other big job notwithstanding--the more time I have to get everything set up just right. It'd be nice to walk in there with a professional-looking information packet and a polished two-minute pitch.
