I'm interested in hearing the answers for LD's questions, as well. It's always fun to check out your log and I wish I had a better idea of what specifically you're doing.

1. I write here my whole workouts.
During the day I do some more reps/sets: pull-ups w/ 90deg legs+weight; airborn lunges on medicine ball; DLs, squats ... ect. and I don't write them.
2. No.
Mostly, I train my clients using rounds (2-3x5; ~10 exercises) and/or Spider Stamina (1 or 2 exercises) and other methods. I pay attention to intensity(!), rest, TUT and to proper(!) form.
3. No.
Only when I show then the proper form of an exercise.

He squats wide, passes a kb under his leg (kb changing hands) and then takes a step. Then he repeats and walks on (in a sumo way).
