It sounds like you have made great progress so far. What is the current program you are on? Have you been doing the same basic exercises for the last 6 months? Take a look at the books New Rules of Lifting or Huge in a Hurry. They have some great programs that are overall full-body compound movements that can concentrate on either weight loss, strength, or muscle mass building. They recommend changing your program about every 6-8 weeks to keep the body from hitting plateaus and they keep the challenges fresh, which means keeping the progress going. I use a specific program for about 6 weeks, then I change to another completely different set of exercises. I don't mix up exercises week to week, I use a specific program for a period of 6 weeks and I hit it hard, (there is often an A, B, and C routine for Monday-Wed -Friday, or whatever). Then after 6-8 weeks I move to another set of exercises. You say you mix up the exercises, so maybe you need to stick with something specific for a while. Your body could be getting used to the same things over and over, but since it is not done repeatedly and intensely for a period of time (6 weeks) you aren't really bombing that exercise and getting a lot out of it. After 6 weeks of bombing your body with those exercises, they begin to adapt and they aren't as effective, so then is the time to hit it with completely new different exercises that your body is not used to.
