A lot of people think that picture is a fake. I honestly don't know, I wouldn't be suprised if it wasn't fake. And many say that was right after his heart surgery, so...who knows. It was taken around the same time as T3, I think a year before, so he looked pretty muscular in T3.
I think it was taken after his heart problems, but before End of Days. He didn't do much around then, and was gone for a while. They never showed him shirtless in End of Days, either, so my guess is that he was still getting back into shape.
