It's 3 sets of 4 reps, and a final set of 12. So, if it was the squats. You would squat 4 reps, rest, 4 reps, rest, 4 reps, rest 12 reps. So you would squat a total of 24 reps.
Some of the other lifts are arranged differently, but follow the same pattern. The number of sets listed, is the total number of sets for that exercise. So, if you see 4 sets of 4,4,8, 12 on barbell push press. It's the first set is 4, second is 4, third is 8, and last is 12.
