To me, I think there is a significant difference. 'MO' is what Alwyn calls it in the book and I believe is simply a type of interval training. HIIT on the other hand, at least the way I understand it, is very short bouts at your maximum effort. Is 'MO' intense and interval based? Sure. Is it HIIT? I'd argue not.
This is were the definitions differ in my head...there is no possible way, as you stated for yourself, anyone could run/bike/swim/etc. at max effort for one minute. Although you're doing interval training, it's not "high intensity" (ie - max effort).
