How did it start?
How long has it hurt?
What makes it hurt more?
What makes it hurt less?
Is it constant?
Is it one sided or both?

What a coincidence! Sport Medicine rounds tomorrow is, "Beyond Core Stability: Evidence based Rehabilitation of Low Back Pain"! I'll post tomorrow morning after rounds.
