When assessing glute function...are we talking about manual muscle testing? or are we having the client perform a glute ham raise and look for the typical compensation from the lumbar spine?

Not really manual muscle testing ... but sort of a modified version I guess. Check the timing of the glutes with a prone hip extension (in many folks the quadratus kicks in FIRST and it should kick in last) ... can they isolate glute contraction (ie a prone isometric glute squeeze) ... etc.

so the knees caving in is from lateral HS's relative to the adductor magnus. But why would there be a muscle imbalance between the two aside from someone that has a static genu valgus? The lateral gastroc has no role in this even though it crosses the knee joint?

What do you mean by toes spread out and knees pointed forward? Are you talking about your feet being externally rotated, while the knees point straight away? It sounds like genu valgus...which could be from body alignment (foot/hip), or tight adductors, medial HS, and internal rotators pulling the knees inwards (my guess on this one).
As Julie said - Gray Cook talks bout it. I have tried using bands, but it didn't work for me. Maybe I used a weak band, maybe it wasn't my thing, maybe there was something else wrong.

I was trying to lead you to external rotation(or whatever) and weak medial HS(compared to latteral HS). You know..I get these some times. I call them "bad English days." Same as a bad hair day, but only shows when I try speaking to someone.
