What do you mean by toes spread out and knees pointed forward? Are you talking about your feet being externally rotated, while the knees point straight away? It sounds like genu valgus...which could be from body alignment (foot/hip), or tight adductors, medial HS, and internal rotators pulling the knees inwards (my guess on this one).

As Julie said - Gray Cook talks bout it. I have tried using bands, but it didn't work for me. Maybe I used a weak band, maybe it wasn't my thing, maybe there was something else wrong.
I was trying to lead you to external rotation(or whatever) and
weak medial HS(compared to latteral HS). You know..I get these some times. I call them "bad English days." Same as a bad hair day, but only shows when I try speaking to someone.

So the main glute dysfunctions are:
- late glutes
- not firing glutes
- weak glutes
- not looking very nice
I read an article at Higher-Faster-Sports a while ago about it, but I guess I'll have to reread it soon.
