Freaky!

J/K ... I have that to some degree too ... not sure what it is ... it doesn't hurt so I don't worry about it! (Also if you saw the size of my feet/ankles during my first pregnancy you'd understand!!)

Could be some micro-tears in the fascia I guess ...

The pain is something else entirely ... feeling like the bottom of his feet are on fire is not normal and honestly probably not related to those bumps. Sounds like either a plantar fasciitis (which maybe it isn't since it sounds like he's been treated for that without success) ... or it could be something with the nerve (adhesions causing impaired mobility ... burning tends to be a nerve-type pain, but not always) ... or it could be referred pain from the subtalar joint (in which case it would respond to some good joint mobilizations, usually into eversion, grade III+ or so ... )

He should just post here and see if we can't hash it out.

The bumpies could be anything really. Lipomas come to mind, or simple cysts...Who really knows from a picture...

We've got a study in press right now (at least I think it's in press) which was a randomized control trial for plantar fasciitis. Orthotics definitely did not come out on top as any sort of definitive treatment.

My guess is that the "squishies" have nothing to do with the symptoms. You see it on a daily basis. Fat pad don't vary much among athletes, fat people, etc.

The foot appears to be pretty rigid to begin with.

Hammer toes laterally and an interphalangeal hallux valgus. All this will make the plantar bony areas more prominent which will be painful.

He may also have capsular problems or tenosynovitis which again is pretty painful and you can't rule out plantar fasciitis because his description is vague.
