Default Shoulder/Lat Issue is driving me insane.

I've been dealing with a right shoulder issue (impingment) for awhile now and have been down many roads for recovery (massage, ART, chiropractic etc.)

A little background...a couple years ago I played in a recreational dodgeball league (most painful of all sports I've ever played) and tore a muscle in my upper back (unsure of which muscle exactly). Not knowing at the time I had done this I played through several weeks of intense burning pain in my upper back (rhomboid area) and referred pain down my arm into my elbow. Fast forward to now and I still have a good sized adhesion in my upper back, a chunk or scar tissue infested muscle. I've had massage therapists hammer on it, electrostim acupuncture etc. It won't go away.

I also went through a period of time about 1 year ago where my upper traps got so tight I was in constant pain and got treatment on that as well. Since the original injury I've been dealing with a burning sensation in the area of that scar tissue, brought on most commonly during the preparation of food. That burning sensation was recently diagnosed as a nerve irritation by my ART practitioner. He also found some tightness in my pecs (not surprising) where there was actual trophic changes in the tissue quality. He described it as grisly...Also noted some tightness in my upper traps, levator scapulae and surrounding cervical spine musculature.

I can feel trigger points, not entirely painful though but big knots (possibly scar tissue?) in the musculature around/on my scapula, including most notably what I'm pretty sure I've nailed down is my teres minor. The muscle travels through the top of my armpit, inserts on my humerus and is intensely irritated mostly by chins or pulls. Whenever I work on my lats they also get very tender and sore in the non-traditional sense, the tissue quality changes as they recover.

I've been doing prehab/rehab work with MM and I/O as well as spending $$$ on treatments but I'm having a heck of a time recovering from this.....I would be forever indebted to you Bill if you could give me an idea of anything really.

What range of motion limitations do you have?

How's your rotator cuff and lower trap strength?

Any ongoing postural issues?

Have the soft-tissue issues resolved after the ART guy worked on them?

Nothing glaring. Tightness in anterior delt though. Performing the over under test I am able to grab fingers with right hand on top and left hand on bottom but vice versa I am quite a distance apart unless I force my right arm into position which obviously I don't do.

Poor. Definitely an issue with the lower traps, ART guy even commented on it.

Weak rear delts and serratus have resulted in winged scapula and abducted forward shoulders. (or so I can say having compared myself to some pics in Sahrmanns Diagnosis and treatment...)

Pain/discomfort subsides for a short period of time but all of the scar tissue and junk is still there.

Bill
Thanks so much for taking a look at this Bill, I'm a huge fan of yours and am looking into the physiotherapy field now much due to your influence.

Just saw the movie "Dodgeball" the other night ... brought back lots of elementary school memories ... I did not know that it was a real thing that adults do!! How fun!!

Anyways ... my opinion is that it would be a good idea to get your lower/mid traps, rhomboids, serratus strong ... it is entirely possible with that degree of weakness that the pain you are experiencing is from that and not from the scar tissue. You won't know until you get it strong ... but if it continues to be weak, you will end up with other problems ...
