What is the proper way of going about this? I have a couple clients who I think have made significant gains (or losses) in most of their cases, and would like to know the proper way of going about asking for a testimonial.
You can also do a customer sastification survey (quick 1 page) as an effort to identify your stregnths and weaknesses. Have an open ended question for comments.

I ask all of the time. There is nothing wrong with it. I just tell them that since they are doing some well here, would they be interested in giving me a written testimonial about the service I have done. You want to make sure you give them exactly what you want and need...kind of a small outline of what you want.

Most of my clients have always gotten excited to be offered a chance to express there experience on paper. Make sure you get a before/after photo and include a release to use teh photos for your own marketing. Most of the time if you have a good working relationship with your successful clients, they don't mind.

Does he have a non-"standard" before photo that he'd let you copy? Like a generic "hanging out at a BBQ being fat/scrawny" photo?
Not quite as good as a measured before photo, but it will still show his progress.

I agree with Isaac. Ask him/her for a photo hanging out at the BBQ or park. The message you want to convey with a before/after pic is the transformation from lazy, sedentary lifestyle to a energetic, healthy one.
