Default Templates for PAR-Q and informed Cons.

So far I've been training in one facility and haven't needed these forms because members sign them as part of membership. That said I'm going to be doing some work in other places and outside once it gets warm, and I was wondering if anyone knows of a place to get general templates for PAR-Q and Informed Consent forms?
Thanks,
Ned

When my new software lauches (some time next century) it will have all of those templates built in.
