It's true that a lot of people who exercise get injured and put a load on health care, specially people who exercise very hard. But I think the real strain on the health care system is the cronic stuff.. Sure, some people who exercise gets some problems that become cronic, but usually it's a one time thing. Other stuff, like obesity causing a host of problems, diabetes II, heart problems, etc, that are all from crappy eating and not exercising probably put a much greater strain on the system. It would be interesting to see some numbers though.. it would actually be a pretty cool epidemiological study to see how much strain is put on the health care system by people who exercise regularly vs people who don't.. just randomize 50k people from the country into each group, make answering the questions mandatory, and we'll have our answer
