You want to do the most weight you can handle with good form for each set. So for the 4,4,4,12 set, after your warm-up sets, maybe you do something like 205,215,205,175. Your first time through will have some trial and error, but then you'll get it dialed in for your next time through.

This is just one example of how you can progress through the sets. You can do something different if it feels more comfortable. Just remember what the ultimate goal is - you want to increase the total volume from one workout to the next.
