There are about 23 Ironman events worldwide, and most of them require you to enter some sort of lottery in order to gain entrance. Once you get in, you pretty much have the spot until you no longer want to race, as they give preference to the folks who have already raced it (ie - 2009 entry opens the day after the 2008 race, and the only slots they put online are the ones that aren't claimed in person).
You're probably right that the "Lottery" is only for Kona. Have you race and Ironman before? If not, I think you'd have a better shot at Kona if you try to qualify via a 70.3 race or a full Ironman elsewhere. I'm not sure how many slots they open for Kona via lottery, but I can't imagine it's very many...
