Ryan, youre on the toes from the very beginning. This is pulling you forward and is causing you to hop back, sometimes enough so that the bar is in front a bit and your butt comes up first when standing from the clean. Its also why some of those lifts were very slow and one of them folded you (think you lost your air too on that one but whatev).
Notice the difference in extensions:
