I use NutribaseEZ on the PC, I've also used LifeForm (PC) in the past.

If I just want to look up a whole food quickly, I use nutritiondata.com or the USDA nutritional database (the database source for the food breakdowns for most of the software out there). USDA offers more portion size options but nutritiondata's interface is a little nicer when giving you the results.
