I can't say about how you should start off your program again...someone more knowledgable will probably answer, but as for taking a couple of weeks off, I know from personal experience, you need to know when you body needs rest and listen to it.
You mentioned you felt burnt out....there's a difference between slacking off and resting. Don't be too hard on yourself. Did you rest between the stages 1 & 2. There is a reason for those rest periods... to avoid burning out.
Get back into the program and look ahead...guilt trips are pretty useless
