You are best off seeing a quality trainer or PT IMO ... self-diagnosis can be a very slippery dangerous slope!

I like Gray Cook's FMS ... perhaps you would benefit from his Athletic Bodies in Balance book as I believe that it was designed more for athletes to help themselves (but having not yet read it I cannot confirm that). His stuff is top-notch.
