Ummm ... yeah ... good point!! It's really not his fault though ... if he knows it is not surgical, and he's a surgeon, then it is really out of his hands.

My initial thoughts in reading this were
1. Lateral meniscus
2. Coronary ligaments (attach the meniscus to the bone)
3. Distal ITB issues

See if he can get a referral to physical therapy ... a good manual therapist should be able to figure it out and treat it appropriately.

This site lists some graduates of their certification program (which is rigorous and tends to produce quality therapists) ... not sure how long you are willing to travel as all of these appear to be about 2 hours from you. (Assuming you are near Harrisburg).

What is up with PA? Every town that I Mapquested came back with TWO different locations ... Mechanicsburg, Doylestown, Willow Grove, Springfield?!?!
