Default Groin pain with piriformis tightness?

Hey all,

My brother is suffering from some severe groin pain, it affects his walking, sitting and standing. Anytime he does any sort of squatting motion, it bothers him. Lunges do not. I've tested him on a few things but nothing is jumping out at me, he does however have a tight piriformis on the same side as his groin pain. I'm guessing there may be a connection here...possibly due to similar insertion points? He also suffers from patellar tendonitis in both knees. Any help would be much appreciated.
