I'm no ART specialist, but I've personally never heard of a muscle strain that is there for life. I could be wrong though.
You keep mentioning muscular things; is anybody saying anything about the spine itself? Because if your problems are all muscular, you can fix it much easier than a spinal problem.
This also jumped out at me: "injured the vastus medialis muscle in September 2006... and have not worked my legs since then." Hell, if your doctor thinks it's about stretching your hams and quads, and that the facet joint is causing it... maybe ask Bill or someone how NOT training your legs may have effected all of this - or if that VM injury lead to the hip problem.
