A: Only fix what's broken. Don't change an exercise for the sake of changing it. If you're making progress then you're making progress...don't mess with it.

B: You're going to stagnate at some point, it's just going to happen and you can't avoid it. Either you outgrow your program and need to specialize a bit, your body just refuses to play any more, or most likely you get bored. I wouldn't overly concern yourself with stagnation until it happens, you never know it could be a week or 10 years.
